Consent to evaluation and treatment. I voluntarily ask Prosper Urgent Care and its providers, nurses, and staff to evaluate and treat me, or the patient I represent. This may include a history and exam, tests, procedures, injections, and prescriptions the provider believes are appropriate.
Risks and no guarantee. I understand that all care has risks and benefits, that no result is guaranteed, and that I may ask questions or decline any test or treatment at any time. I have given complete and honest information about my health, medications, and allergies.
Not emergency care. Urgent care is for non-life-threatening problems. If my condition gets worse, or I think I’m having an emergency, I will call 911 or go to the nearest emergency room. I understand I should follow up with my primary care provider or a specialist as advised.
Payment. I’m responsible for the price of the services shown at check-in, due at the time of service.
Prescriptions and medication history. I allow my provider to review my medication history and to send prescriptions electronically to the pharmacy I choose.
Privacy (HIPAA). I have been offered Prosper Urgent Care’s Notice of Privacy Practices. It explains how my health information may be used and shared for treatment, payment, and health care operations, and it describes my rights, including the right to see my records, ask for corrections, and ask for restrictions. I may ask for a paper copy at any time. Signing acknowledges that I received the notice. It does not waive any of my rights.
